From this book, I learned something such as the difference between safety and reliability. However, the real useful content of this book seems to occupy only a small portion in terms of volume. Too many repetitions of same explanations of STAMP concept bothered me. In addition, explanations tend to be too qualitative in nature. I hope the author or other people implementing the idea write completely new book, concentrating real applications in the near future.
If you're interested in safety .. or just good design practices in general, this book is a must read. It's very long and very winded but that's my only complaint. A definitive guide on STAMP and STPA.
I agree with reviewer #1 that this isn't a "how to" book. That said, this book should be mandatory reading for all engineers and patient safety experts in healthcare (design, IT, clinical, standards, regulatory, etc.). In addition to the outstanding content, one of the things I liked best about it is that it's digestible for non-engineers. Bravo!

Solid refudiation of the bureaucratic paper drills that are all too common today. Followed by a structured, methodical, effective process that should supplant much of what passes as safety analysis. Must-read.
Leveson's treatment of the accident system as a intergrated human process is brilliant.This book should be required reading for everyone in leadership for risky processes and everyone in technical/process safety positions.
